We’re here to Support You
Every Step of the Way
At Citizen Care Australia, we provide personalized support for participants, families, and cares.
A Family Support Plan plays an important role in early intervention by recognising the vital role families have in a child’s development. By actively involving parents and caregivers, therapy strategies can be applied not only during sessions but also throughout daily routines at home and in the community.
This collaborative approach helps families gain confidence, strengthen everyday routines, and better manage challenges such as behaviour in social or public environments. A well-structured support plan equips families with practical strategies, guidance, and emotional support so they can effectively assist their child’s development. Supporting both the child and their family encourages consistent progress and long-term positive outcomes.
A Functional Capacity Assessment (FCA) focuses on understanding how a child communicates their needs, emotions, and daily challenges. The goal is to identify practical communication strategies that match each child’s abilities and developmental stage.
Through personalised methods such as visual supports, assistive technologies, and structured learning techniques, FCA helps improve communication and social interaction while reducing frustration. These strategies allow children to express themselves more effectively, build confidence, and participate more comfortably in everyday activities.
By strengthening communication and independence, FCA support encourages meaningful progress in both emotional wellbeing and social development.
An End of Plan Review evaluates the progress achieved during your child’s current support plan. It provides a clear overview of improvements, strengths, and areas that may require additional focus.
This process helps families and therapists recognise milestones while also identifying new goals or strategies that can support ongoing development. Reviewing outcomes ensures the support plan continues to evolve in line with the child’s changing needs.
As children grow and develop, their abilities and support needs may change. A Change of Circumstances review allows the support plan to be updated so it continues to reflect the child’s current situation and developmental goals.
Both standardised and non-standardised assessments can be used to evaluate progress and provide deeper insights into a child’s strengths and challenges. These evaluations help therapists design personalised strategies and ensure the support provided remains effective and relevant.
